A hash whose message matches an existing issue once numbers, ids and quoted text are blanked joins that issue instead. Each issue carries a fix prompt to paste into Claude Code. Issues closed as completed reopen when the error shows up on a newer place version of PLACE; closed as not planned (or duplicate) mutes the error for good.
